GHS Sport Physicals
On Saturday, May 10, 2025, Lexington Family Practice will be on campus beginning at 8:00am to conduct sport physicals by appointment in the GHS Gym. There will be limited spots available.
Tickets will be $20, plus processing fee, from April 1st until May 2nd at 12:00pm.
Tickets will increase to $30, plus processing fee, beginning May 2nd at 12:01 pm until May 8th at 11:59 pm.
All payments are NON REFUNDABLE.
PHYSICALS for RISING 9th - 12th GRADERS ONLY.
Please Note:
Be sure to bring a completed SCHSL Physical Form at the time of the physical. It can also be found at gilbertathletics.org >Sports Medicine.
Wear Shorts, T-Shirt and Flip Flops.
All physicals must be dated after April 1, 2025 to be valid for the 2025-26 school year.
Reminder current physicals will expire at the end of the 2024-25 school year (June 30th).
No athlete may participate in a GHS sport without clearance in Big Teams/PlanetHS (includes strength/conditioning sessions, open season workouts, tryouts, practices, etc).
